Depression is a common response among cancer patients to their diagnosis and treatment; however, it goes undetected by healthcare providers in about 50% of cases. Communication skills training has been suggested as means to help nurses detect and respond to patient depression. We developed and pilot tested a communication skills training workshop based around 6 strategies. The training program consisted of 2 half-day experiential workshops that included didactic teaching, exemplary video, and role play. The aim of the study was to evaluate the effectiveness of the communication skills training. Fifteen nurses were recruited from the ambulatory nursing service at (redacted). Standardized patient assessments were used to measure strategy uptake.

Depression ist eine häufige Reaktion von Krebspatienten auf ihre Diagnose und Behandlung, allerdings geht es unbemerkt von Gesundheitsdienstleistern in etwa 50% der Fälle. Kommunikationstraining ist vorgeschlagen worden, als Mittel zu helfen Krankenschwestern erkennen und darauf reagieren zu Patient Depression. Wir entwickelten und getesteten Piloten eine kommunikative Fähigkeiten Lehrwerkstatt rund 6 Strategien. Das Trainingsprogramm bestand aus 2 halbtägige Workshops, die auf Erfahrung didaktischen Lehr-, exemplarische Video-und Rollenspiele inklusive. Das Ziel der Studie war es, die Wirksamkeit der Kommunikationstraining bewerten. Fünfzehn Krankenschwestern wurden aus dem ambulanten Pflegedienst an (geschwärzt) rekrutiert. Standardisierte Patienten Einschätzungen wurden verwendet, um Strategie-Aufnahme zu messen.

During the 5-year study period, blood pressure measured in the outpatient clinic with the use of auscultory or oscillometric techniques, glomerular filtration rate, and urinary protein excretion were assessed every 2 months, and ambulatory blood-pressure monitoring was performed every 6 months. Antihypertensive therapy was adjusted according to levels of 24-hour mean arterial pressure to achieve the target blood-pressure levels. Any antihypertensive agents except for other antagonists of the renin–angiotensin system were allowed to be added to the patient's drug regimen. A standardized antihypertensive drug-escalation protocol was suggested but was not compulsory (see the Supplementary Appendix).

Während der 5-jährigen Studiendauer wurden Blutdruck in der Ambulanz mit dem Einsatz von auscultory oder oszillometrischen Techniken, GFR, und Proteinausscheidung im Urin gemessen beurteilt alle 2 Monate, und ambulante Blutdruck-Monitoring durchgeführt wurde alle 6 Monate. Antihypertensiven Therapie wurde nach Ebenen der 24-Stunden eingestellt mittleren arteriellen Druck, um die Ziel-Blutdruck zu erreichen. Jeder antihypertensiven Therapie mit Ausnahme von anderen Antagonisten des Renin-Angiotensin-System durften, um den Patienten medikamentöse Behandlung hinzugefügt werden. Ein standardisiertes blutdrucksenkende Arzneimittel-Eskalation Protokoll wurde vorgeschlagen, war aber nicht zwingend (siehe Ergänzende Anhang).

Quality management and organisational development activities can address horizontal (inside the hospital) or vertical (between hospital and ambulatory care) communication problems within the healthcare system, can analyse the reasons for them and develop solutions (Schaufeli and Enzmann 1998). Better communication within the hospital is impeded by recent developments such as the reducing length of stay in hospitals without adjusting the offer of ambulatory care, staff shortening in oncological or palliative care units, cutting reimbursement and “outsourcing” of psycho-social services.

We tested the hypotheses that (1) high pessimism and low optimism (LOT-R overall and subscale scores) would predict high ambulatory blood pressure (ABP) level and 24-hour load (percentage of ABP values exceeding the pediatric 95th percentile) among healthy Black and White adolescents (n=201; 14-16 yrs) across 2 consecutive school days and (2) that the relationships for the pessimism and optimism subscales would show nonlinear effects. The hypotheses were confirmed for pessimism but not for optimism. The results suggest that high pessimism may have different effects than low optimism on ABP and that even moderate levels of pessimism may effect blood pressure regulation. These results suggest that optimism and pessimism are not the opposite poles on a single continuum but ought to be treated as separate constructs.

Wir testeten die Hypothese, dass (1) hohe Pessimismus und niedrigen Optimismus (LOT-R Gesamt-und Subskalen) würden hohe ambulante Blutdruck (ABP)-Ebene und 24-Stunden-Belastung (Prozentsatz der ABP-Werten oberhalb des pädiatrischen 95. Perzentile) bei gesunden vorherzusagen Black and White Jugendlichen (n = 201; 14-16 Jahre) in 2 aufeinander folgenden Schultagen und (2) dass die Beziehungen für den Pessimismus und Optimismus Subskalen würde nichtlineare Effekte zu zeigen. Die Hypothesen wurden zum Pessimismus, aber nicht für Optimismus bestätigt.
