The GALT gene is located on chromosome 9p13 and consists of 11 exons. Over 230 mutations have been described. The most frequent mutation in the Caucasian population, with an overall frequency of 65% (96% in the Irish population), is the Q188R mutation, which results in a complete loss of enzymic activity. The second commonest European mutation is the K285N, a missense mutation, which predominates in central European counties. This also results in a complete lack of GALT activity. In contrast, S135L, which accounts for 50% of mutate alleles in African Americans, shows near normal activity in mouse models. Whilst there is a relatively good correlation between genotype and residual enzymic function, the correlation between genotype and clinical phenotype is more enigmatic, though Q188R is predicative of a poorer clinical outcome, whereas S135L is associated with the milder phenotype seen in Afro-Caribbean patients.
