The treatment of choice for end-stage solid-organ failure in children and adolescents in most cases is organ transplantation (1). Much of the success of solid-organ transplantation is reflected in the long-term survival rates of both the patient and the allograft (Table 1), which can be attributed to several factors including advances in surgical and organ preservation techniques, improved immunosuppressive and antiviral regimens, and changes in donor organ allocation. Several pediatric renal transplant cohorts demonstrate patient survival well above 90% at five and 10 yr and above 70% at 20 yr (2–4). With this success, children and adolescent transplant recipients are confronted with new challenges that arise as long-term sequelae of transplantation. Transplantation in essence replaces end-stage disease with a more sustainable chronic disease state and as such has profound clinical and psychosocial consequences.
