The interproximal gingival papilla fill was assessed according to an index. Pathosis, pulp obliteration, root length, and crown-root ratios were studied on standardized radiographs. The results showed that the clinical variables for transplants did not differ from those of the natural incisors, except for some increased mobility and more plaque in a few transplanted premolars. The interproximal gingival papillae adjacent to all transplanted teeth were normal or slightly hyperplastic, and no interdental gingival recession (black triangles) were seen. As expected, all transplants showed varying degrees of pulp obliteration. The findings also demonstrated that tooth transplantation has an inherent potential for bone induction and reestablishment of a normal alveolar process.

Interproximal Zahnfleischpapille Füllung wurde gemäß einem Index bewertet. Pathosis wurden Zellstoff Auslöschung, Wurzellänge und Kronen-Wurzel-Verhältnis auf standardisierten Röntgenaufnahmen untersucht. Die Ergebnisse zeigten, dass die klinischen Variablen für Transplantationen nicht von derjenigen der natürlichen Schneidezähne unterscheiden sich, abgesehen von einigen mehr Mobilität und mehr Plaque in wenigen transplantierten Prämolaren. Die interproximalen Zahnfleischpapillen neben allen transplantierten Zähne waren normal oder leicht hyperplastische und kein interdentalen Gingiva (schwarze Dreiecke) wurden gesehen. Wie erwartet, zeigten alle Transplantationen in unterschiedlichem Maße von Zellstoff Auslöschung. Die Ergebnisse zeigten auch, dass Zahn-Transplantation eine inhärente Potenzial zur Induktion Knochen und Wiederherstellung eines normalen Alveolarfortsatzes hat.

When 2 or more neighboring incisors are missing, a combination of premolar autotransplantation and orthodontic space closure might be the optimal treatment alternative. As discussed elsewhere,6 careful detailing throughout the orthodontic progress and finishing stages to achieve optimal positioning and crown inclination of all teeth, coupled with new techniques and materials adapted from esthetic dentistry can, even in these difficult treatment situations, restore natural tooth shapes and sizes and provide normal gingival texture and contours around all the teeth.

For those age groups, we have to discuss alternative orthodontic space closure and prosthetic replacement as implant placement. Both of them are well accepted by some patients but absolutely not by others, depending on the aesthetic outcome. Before discussing advantages versus disadvantages in cases with missing maxillary incisors or mandibular premolars, we have to stress that

A search in the MEDLINE (Pub Med) database from Jan 1990 to June 2007 was made. The search included the key words: missing maxillary incisors AND orthodontics OR dental implants; missing premolars AND orthodontics OR dental implants. Totally 146 articles were found. Only studies in the English language or other languages with an English abstract were accepted, which reduced the number to 91articles. Among them, one prospective and three retrospective studies, two review-papers and five case reports with a discussion were found among them. The others presented one or two cases, or focused on treatment planning of missing teeth in general and were thus excluded.

Eine Suche in der MEDLINE (Pub Med)-Datenbank von Januar 1990 bis Juni 2007 hergestellt wurde. Die Suche berücksichtigt die Schlüsselwörter: fehlende oberen Schneidezähne und Kieferorthopädie oder zahnmedizinischen Implantaten; fehlende Prämolaren und Kieferorthopädie oder Zahnimplantaten. Total 146 Artikel wurden gefunden. Nur Studien in englischer Sprache oder anderen Sprachen mit einer englischen Zusammenfassung wurden akzeptiert, die reduziert die Anzahl an 91articles. Unter ihnen wurden einer prospektiven und retrospektiven Studien drei, zwei Kritik-Papiere und fünf Fallberichte mit einer Diskussion unter ihnen gefunden. Die anderen präsentierten ein oder zwei Fälle, oder konzentrierte sich auf die Behandlungsplanung von fehlenden Zähnen im Allgemeinen und wurden somit ausgeschlossen.

Space closure Replacing a lost central incisor by moving a tooth over the inter-maxillary suture was shown to be unsuccessful (14). Hence, the gap has to be closed from both sides, resulting in three incisors. Some patients may accept such an alternative, if the incisors are of the same size and their axial inclination had been corrected. Implant treatment seems to be to the best choice in such cases.

Lückenschluss Ersetzen eines verlorenen mittleren Schneidezahn durch Bewegen eines Zahnes über die inter-maxillaris Naht erwies sich als erfolglos (14). Daher hat der Spalt von beiden Seiten geschlossen werden, was zu drei Schneidezähne. Bei einigen Patienten kann eine solche Alternative zu akzeptieren, wenn die Schneidezähne von der gleichen Größe sind und ihre axiale Neigung korrigiert worden war. Implantat-Behandlung scheint sich auf die beste Wahl in solchen Fällen sein.

Space closure which aims at placing a canine to substitute missing lateral incisors often is associated with aesthetic problems. Today, however, this procedure has become a more popular treatment choice from periodontal aspect (15). Opponents on the other hand point to compromised dental aesthetics, high relapse tendency, and lack of canine protected occlusion. However, no significant difference in prevalence of temporo-mandibular disorders was noticed between patients treated with space closure or prosthetic replacement (16).

Lückenschluss, die bei Platzieren eines Hundes, um fehlende seitliche Schneidezähne ersetzen soll oft mit ästhetischen Problemen verbunden. Heute jedoch hat dieses Verfahren zu einem immer beliebter Behandlung von parodontalen Wahl Aspekt (15). Gegner auf der anderen Seite weisen auf kompromittierten Zahnästhetik, hohe Rückfallquote Tendenz, und der Mangel an Hunden geschützten Okklusion. Es wurde jedoch kein signifikanter Unterschied in der Prävalenz von Kiefergelenk-Erkrankungen zwischen Patienten mit Lückenschluss oder prothetischen Ersatz (16) behandelt bemerkt.
